Completing the Application

You may:

1) Complete the application online, sign with a digital signatureand email to sararotary@aol.com
2) Complete the application online, print, sign and mail or hand deliver to the club's Executive Secretary
3) Print the application, complete, sign and mail or hand deliver
4) Do not complete the Sponsor information on page 4

Tips for Completing the Application Online

1) Enter all required information
2) Hovering your mouse over some of the fields will provide additional help
3) Use the tab key to easy navigate through the fields
4) Sign where indicated on pages 2 and 3

Digital Signatures

Attach your digital signature where requested at the bottom of pages 2 and 3. To do this, select "Sign" in the toolbar at the top of your Acrobat Reader© program. If you do not have a digital signature on file, the program will walk you through obtaining one. It is a very simple process that takes just a couple of minutes. You will need to create a password that is easy to remember. Once your digital signature is obtained, you simply click the "Sign"tool, scroll down to the location requiring your signature, and by holding the right mouse button down, create a box in the area you wish the signature to appear. Each time you wish to imbed a digital signature, you must provide your password. Once a document is signed, it cannot be altered or changed.
